As you fill out this form, you will be guided to upload essential documents to verify your proof of BC residency and proof of Canadian citizenship, as well as other supporting documents. 
Please provide: 

• Proof of BC residency (POR), such as BC Driver’s License, BC Medical Services card, BCID or lease / rental / purchase agreement. 
  • For students under 19-years-old, POR must show the parent / guardian’s proof of residence.  
  • For students 19-years-old and over, POR should be in the student’s own name (even if still living at home). 
     
• Proof of citizenship documents such as Canadian passport, Status Card, Permanent Resident card, Landed Immigrant Papers or Canadian birth certificate. 
   
• Recent report card or transcript. 
   
• Individualized Education Plan (IEPs) if applicable.
   
• Proof or name change or marriage certificate if applicable. SIDES requires all the previous names held by students. 
   
• Custody agreements if relevant.
